This form serves as a legal document that details the goods that you are shipping. This form also works as a receipt that can be issued to customers once they have received their shipment.

Signatures On This Form

This document will have to be signed throughout various points of the shipping process. First, the people shipping the goods will sign it. The shipping company will then sign it and lastly, it will be signed when the recipient receives the goods.
